7th & 8th Grade Registration

All families who wish for their student participate in athletics MUST have the following ONLINE forms completed PRIOR to the paperwork deadline due date for their athlete's sport (please allow a minimum of 48 hours for the processing of paperwork)

These forms must be completed once each academic year for athlete registration with the district...

1. Digital Athletics Packet (This is the district's registration packet completed once per year through the Informed K12 system)

2. Annual Sports Physical Form [Click here for Spanish]

***Athletes MUST have an annual physical form signed and stamped by a medical doctor. The form can be attached on the Informed K12 Digital Athletics Packet. If necessary, a copy can be turned into the Athletics Box in the Emerson Office.

***The sports physical on file must have been conducted within a year of any athletic activity. If your student's physical will expire during their current season, a new form will need to be completed before the date of expiration for the student to continue participation in athletic activities.

Athletes should complete an Emerson/Da Vinci Google sign-up form for each team they would like to join throughout the year...

1. Google Sign-up Form - This form is used to build team rosters and contacts lists. Please ensure contact information is filled in correctly before submitting. If this form is not completed prior to the paperwork deadlines listed below, you will not be added to the team.

Parents MUST complete the K12 Digital Athletics Packet & have submitted a signed physical to the Emerson/Da Vinci offices by the date listed next to the athlete's sport.

Athletes MUST have signed up for the team using the Google sign-up forms linked above.
