We are happy to announce that we have transitioned to FinalForms for our document management system for athletics. This is the same platform used by Davis Senior High and your account will transfer over to the DSHS Athletic Director when you move to 9th grade Athletics.

If you are interested in participating in Junior High Sports this year, please follow the following steps to sign up to participate:

Before going to the doctor, print out a Sports Physical Form (English) (Spanish) for your doctor to sign. Hard copies of this form are also available in the Emerson office.

Athletic Donations

We ask for a $25 donation per student per sport. This donation helps to partially cover the cost of referees, equipment and uniform replacement, fees associated with the league as well as the cost of the use of the FinalForms platform ($5/student).
